Create and customise a Form

Choose a Form starting point, add useful fields, and control what happens after someone submits.

A Form lets someone give you an answer without leaving your Reroute page. Use it for enquiries, signups, feedback, phone details, short questionnaires, or recorded voice answers.

Choose the right starting point

Open Add block and choose the Form option that best matches the result you need.

Starting pointUseful when

A phone number alone does not create an audience Contact. Add an email field whenever the response should also create or update a Contact.

Give the Form a clear job

Name the Form after the visitor’s intended result. Ask about a wedding booking is clearer than Contact us when the page is about wedding photography.

Add a short description when someone needs to know what information to provide or when they can expect a reply. Avoid promising a response time you cannot reliably meet.

Add only useful fields

Open the Form editor to add and arrange the information you need.

Available field types include:

  • name, email, and phone;
  • short answer or paragraph;
  • single choice, checkboxes, or dropdown;
  • date; and
  • audio recording.

Use a choice field when the visitor can pick from a known set of answers. Use a short or long written answer when their context is more important than consistency.

Mark a field as required only when you genuinely cannot complete the visitor’s request without it. A smaller Form is usually easier to finish.

Capture a fictional enquiry Form with a name, email, and short-answer field already configured, then show the Add field picker with identity, written-answer, choice, date, and audio-recording options. A side-by-side or stepped capture is fine.

Framing: Include the selected Form, its ordered field list, and the field-type selection view

Use email when you need a Contact

Every successful submission becomes a Response. An email address is what lets Reroute create or update a Contact for the current page.

A name, phone number, anonymous answer, or audio recording can still be part of a valid Response without creating a Contact. If the same email appears again on the same page, its new Response joins the existing Contact history.

Email collection and marketing consent are different. Use an intentional Email signup or Subscribe flow when someone is agreeing to future updates.

Record a voice response

Choose Voice messages or add an audio recording field when the visitor’s spoken answer is important.

Recordings can be up to 10 minutes on both Free and Creator. Test the Form on a phone and allow browser microphone access when prompted.

A recorded answer is stored with its Response. When response email notifications are enabled, the notification can include the recorded audio.

Configure the submit and confirmation steps

The Form editor also lets you adjust the visitor experience after the fields:

  • Submit button controls the action label.
  • Terms controls the disclaimer shown near submission.
  • After submit controls the confirmation message and any redirect.

Use an action such as Send enquiry or Join the list when it better describes the visitor’s intent than a generic Submit.

Creator can link custom terms in the Form disclaimer. On Free, the normal Reroute disclaimer remains in place.

If you add a redirect, open the destination yourself and check that it works without an unexpected login.

Open: /app/page

Framing: Include the complete visitor-order editor and the currently selected confirmation or terms settings

Submit a real test

  1. Open the public page outside the editor.
  2. Complete the Form with fictional information and an email address you control.
  3. Confirm the success message or redirect.
  4. Open Responses and locate the submission.
  5. Check Contacts when the submission included an email.

For filtering, reading, and exporting the resulting information, continue with Manage responses and contacts.
