Use a Form to collect an enquiry, written answer, signup, phone detail, or recorded voice answer. Every submission is stored as a Response. A submission with an email address also creates or updates a Contact for that page.
Add a Form
Open the page editor, select Add block, then choose Form.
Give the Form a title that tells visitors what they are responding to. Add a short description when the request needs context, then choose the fields you need.
A field can collect a name, email address, phone number, written answer, choice, date, or recorded voice answer. Ask for less when you can. Every extra field gives someone another reason to stop.

Once the Form has its required information and the save completes, it appears on the public page. There is no separate Publish step.

Decide whether you need an email address
Add an email field when you need a durable Contact or plan to reply outside the original submission.
A Response without an email address is still complete and useful. It remains in Responses, but it does not create a Contact. A phone number on its own also does not create one because Contacts are keyed by email.
Use a Subscribe signup when the visitor is explicitly asking to receive future updates. A general Form that happens to collect an email does not automatically mark that person as subscribed.
Submit a test response
Open the public page rather than relying only on the editor preview.
- Complete the Form with an email address you can recognise.
- Submit it and check the confirmation state.
- Open Audience, then Responses, and find the new submission.
- Open Contacts and confirm the email appears there.
This one test checks the visitor experience and the creator workflow at the same time.

Understand Responses and Contacts
Audience contains 2 related views.

If the same email appears in several Forms on one page, those submissions remain separate Responses and resolve to the same Contact. The same person submitting on 2 different Reroute pages appears as a Contact on each page.

Know what each plan includes
Response capture is unlimited on every plan. Visitors can keep submitting and Reroute keeps every Response.
On Free, you can manage the newest 10 Responses and their related Contacts. Older history becomes locked but is not deleted. Creator unlocks the complete history and CSV export.

New Responses remain available
The view allowance keeps the newest Responses open. A new submission never disappears behind older history.
If something looks wrong
The Form is in the editor but not on the page. Open the Block and complete the required information named on its card.
A Response did not create a Contact. Check whether the submission included a valid email address. Name and phone fields cannot identify a Contact by themselves.
A Contact is not marked subscribed. Subscription records the visitorβs intent. Use a Subscribe signup or an Email signup Form when the visitor is agreeing to future updates. Understand subscribers and consent explains the distinction.
Older rows are locked. Capture has continued. Creator unlocks the complete history and export.
Check the Form
- The title explains what the visitor is responding to.
- Every field is required for the response or follow-up.
- An email field is present when you need a Contact.
- A public test appears in Responses and Contacts as expected.
